With the collaboration of your primary care provider I can offer you Medical Nutrition Therapy on various diseases and disorders. ---------------------------------------------------------------------- What is Medical Nutrition Therapy (MNT)? It is an evidenced-based approach that involves a nutritional diagnosis, therapy, and counseling services for the purpose of managing certain chronic diseases and disorders, through the use of an individualized diet plan. The nutrition plan is ordered by your primary care physician and implemented by a registered dietitian (RD). MNT involves in-depth individualized nutrition assessment and a duration and frequency of care using the Nutrition Care Process to manage disease. MNT may reduce complications associated with chronic disease such as in Diabetes. Includes an initial consultation, single follow up session, exclusive handouts and educational material, along with email support for 1 month Conditions: -Diabetes -Hypertension -Heart Disease -Obesity -IBS/IBD/SIBO -Endocrine Disorders - Hypo/Hyper Thyroid, Hashimotos, Hormone Imbalance All consultations are conducted virtually via ZOOM. Appointment reminder email will be sent with details to join session. * Sessions cost $165. Full balance of payment is required at time of service provided. * (Payments Accepted: Checks made payable to NitaNutrition LLC or Venmo, Credit Card - Invoice sent to email).
